Your membership is effective immediately!

Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we’ll make sure your membership is current, and you will be eligible for instant savings off our regular fees.

    If you’re a current patient, please contact us to confirm you’re on Perio Maintenance. If you are a new patient, we will make a recommendation after your initial exam.

Your club membership is solely with our office and as such is non‑transferrable. If you move to another area, you may cancel your membership at any time.

    Simply call us and let us know. No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.

    You’ll pay only the Lifetime Activation fee to join. This fee includes your first month’s membership dues. Memberships may be paid month‑to‑month or annually.

An email address for receipts, credit or debit card, full name, address, phone and date of birth.

It will be your responsibility to reschedule. Please see our office policy regarding no‑shows and last-minute cancellations. To schedule, simply call our office and setup an appointment for yourself or your family members.

This program is a preventative membership plan, not a dental insurance plan.  It cannot be used:

• with any dental insurance plan.
• for treatment which, in the sole opinion of the treating doctor, lies outside the realm of the doctor’s capability.
• for referrals to specialists.
• for fees paid to specialists or labs

• Monthly fee is due on the 1^st day of the month
• if your plan lapses, you will be required to pay the Lifetime Activation fee plus five months dues to rejoin.
• No refunds will be issue at any time if participant decides to not utilize the dental plan.
• Payment for services beyond Preventative are due at time of treatment.

Here are some of the procedures and products Apollonia Smiles is not able to offer a discount on and their prices:

• Oral Cancer Screenings $25/yr
• MI Paste (Plus) $30
• Panoramic/CBT Scan/3D imaging $100
  • Does not include fee for an Oral Radiologist Review if requested
• Invisalign, Candid, or other Clear Aligner Therapy (Price depends on Treatment Plan)
• Late cancellation/no show fees $75/hr of appointment
• Sleep and Breathing treatments such as, but not limited to:
  • Vivos, Myofunctional Therapy, Solea Sleep (Price depends on Treatment Plan)
